Team Staff Qualifications Confirmation
This form must be completed and submitted with the required TWO screenshots from your SPORDLE account before an individual can be added to a team roster. Please give your full name as it appears on your certifications and provide the age category and Head Coach for ONE team to which you are being rostered. There is no need to enter multiple teams - the same information is used for all teams to which you are rostered. For details as to how to obtain the necessary screenshots please refer to the posted Team Staff Registration document, Step Seven.
